Waiver of Liability

A Release of Liability must be signed before participating in any avalanche training program. Winter mountain activities involve many risks, dangers and hazards.

Be sure to include the mm/dd/yyyy and nearest Town, Province for the field date that you plan to attend.
Your instructor is required to witness all participant waiver forms.
